Just remember that the true anticoagulant effect is delayed by 48–72 hours! 🎯 Dosing & Monitoring: • Start with an initial dose of 2–5 mg/day, but reduce this for elderly patients, those with hepatic impairment, or low albumin. • Aim for an INR target of 2–3 for most indications, or 2.5–3.5 for mechanical mitral valves. • Your goal? Keep that Time in Therapeutic Range (TTR) >=70%! ⚠️ Crucial Watch-Outs: • Interactions: Watch out for potentiators like amiodarone and macrolides, inhibitors like St. John's Wort, and NSAIDs which increase bleeding risk. • Diet: Consistency is key!
