Diagnosis of Pleural Membrane Disorders by X-ray ☢️🩻 تشخيص إصابات الغشاء البلوري بالأشعة ☢️🩻 ​1. Hemothorax🩸 ​تراكم الدم في التجويف الجنبي، ويظهر في الأشعة كعتامة متجانسة (Radio-opacity) في قاعدة الرئة مع اختفاء الزاوية الضلعية الحجابية. ​Accumulation of blood within the pleural cavity, appearing as a homogeneous radio-opacity at the lung base with obliteration of the costophrenic angle. ​2. Pneumothorax 🫁💨 ​وجود هواء في التجويف الجنبي يؤدي لانهيار الرئة؛ يظهر في الأشعة كمنطقة شديدة السواد (Lucency) مع غياب لعلامات الأوعية الدموية وخط جنبي مرئي. ​Presence of air in the pleural space causing lung collapse; radiographically characterized by a hyperlucent area with absent vascular markings and a visible visceral pleural line. ​3. Pleural Effusion💧 ​تجمع سوائل غير طبيعي في الفراغ الجنبي؛ تظهر علامة "السطح الهلالي" (Meniscus sign) بوضوح في صور الأشعة السينية للصدر. ​Abnormal collection of fluid in the pleural space; typically demonstrates the characteristic "Meniscus sign" on upright chest X-rays. ​4. Atypical pneumonia and cardiogenic pulmonary edema can look very similar on chest X-ray. You cannot always distinguish them from imaging alone, but distribution, heart size, pleural effusions, and clinical context help significantly. Save this. 🫁 Reticular Markings on Chest X-ray — How to Read Them 🔍 What does “reticular” mean? • A network of fine lines forming a mesh or lattice pattern • Represents thickened interstitial structures (septae, fibrosis, or inflammation) 📸 How it appears on CXR • Multiple thin linear opacities • Often bilateral and diffuse • Can be basal-predominant or peripheral 🧠 Pathophysiology (why it happens) • Thickening of the interlobular septa • Interstitial edema, inflammation, or fibrosis • Not an air-space process → alveoli usually spared 📋 Common causes to think of • Interstitial lung disease (ILD) – e.g. pulmonary fibrosis • Pulmonary edema (especially early / interstitial phase) • Chronic infection (e.g. TB, viral pneumonitis) • Connective tissue disease–related lung disease • Lymphangitic carcinomatosis 🚨 Red flags on CXR • Progressive reticulation • Reduced lung volumes • Subpleural or basal predominance • Associated honeycombing (suggests fibrosis) 🩻 Next best step • HRCT chest → gold standard for characterising interstitial disease • Correlate with clinical history + exam + labs 📝 Take-home message ➡️ Reticular markings = interstitial pathology ➡️ CXR gives a clue — CT gives the diagnosis #pulmonology #radiology #doctor #medicine #medicalstudent
