Clinical examination of ascites focuses on confirming the presence of fluid in the abdomen and assessing its severity and cause: 👀 1. Inspection Look at the patient from the foot end and side: Distended abdomen with bulging flanks Full, tense abdomen (in large ascites) Everted umbilicus Skin changes: Dilated veins (seen in portal hypertension from Cirrhosis) Stretch marks (striae) Movement with respiration may be reduced In massive ascites, abdomen looks like a “frog belly” ✋ 2. Palpation Fluid thrill (fluid wave test) Assistant presses the midline to prevent transmission through fat Tap one flank → feel impulse on the opposite flank Suggests large ascites Shifting dullness (also partly percussion-based) Palpation helps confirm fluid movement Organ palpation Liver and spleen may be difficult to feel in tense ascites After fluid removal, hepatomegaly or splenomegaly may become evident Check for tenderness (may suggest infection like Spontaneous Bacterial Peritonitis) 🔊 3.
