A crew member replied that they could see the airliner, and requested "visual separation" from the plane; this was approved by the controllers. A few seconds before the collision, the air traffic controller ordered the helicopter to pass behind Flight 5342. The incident was captured by a webcam at the John F. Kennedy Center for the Performing Arts. Another video shows a brief trail of fire. On January 29, 2025, American Airlines Flight 5342, a PSA Airlines CRJ700, collided midair with a U.S. Army UH-60L Black Hawk helicopter over the Potomac River. The accident occurred just southeast of Ronald Reagan Washington National Airport (DCA) as the regional jet was on its final approach to Runway 33. All 67 people aboard both aircraft were killed, making it the deadliest U.S. aviation disaster since 2001. A yearlong investigation by the NTSB, finalized in January 2026, determined the crash was caused by “deep, underlying systemic failures” rather than a single error. Key factors included the FAA’s flawed placement of a helicopter route directly beneath a busy commercial approach path and an over-reliance on the “see and avoid” concept. Additionally, a high workload in the DCA control tower and a faulty altimeter in the Army helicopter—which led the pilots to fly higher than intended—contributed to the fatal lack of separation. In the wake of the tragedy, the FAA permanently closed the problematic helicopter route and implemented stricter staffing and arrival rate mandates at DCA. New federal legislation, known as the Rotor Act, was introduced to close safety loopholes and mandate better position-sharing technology (ADS-B) for both military and civil aircraft in congested airspace. The Justice Department has since admitted liability for the accident, citing failures by both the military crew and air traffic controllers to maintain proper vigilance. #DCACrash #AviationSafety #NTSB #dailysscaries #Flight5342
