This is a cool in-progress result of an amazing person who came to see me with left facial weakness and complete lack of smile - the left corner of the mouth does not elevate. We first attempted Botox with facial retraining. Chemodenervation did not result in ANY elevation of the corner of the mouth or change her smile dynamics at all. On exam, I noted that she has a small left cheek. This reflects the lack of muscle strength in the zygomaticus muscles that are required to produce a smile. Given this, and her lack of response to Botox, we decided to add strength to this area of the face with a free gracilis powered by the trigeminal nerve. Her surgery was successful and she is already producing a nice smile about 5 months out from surgery.
