Ever wonder why one side of your back always feels tighter than the other, even when you stretch both sides equally? It’s usually your hips. One hip is usually tighter than the other. Most people have no idea which one it is until they do this test. What you’re seeing is the FABER test (Flexion, Abduction, External Rotation). It’s one of the first tests I do in clinic to check for hip asymmetry and restrictions that cause lower back pain. How to do it: 1. Lie on your back 2. Bend your right knee 3. Place your right ankle on your left knee (figure 4 position) 4. Notice how far your right knee drops toward the floor 5. Repeat on the other side 6. Compare the two sides What it means: Whichever knee doesn’t drop as far is your tighter hip. That’s the side with restricted mobility. Hip range of motion asymmetries may be one of the causes of non-specific lower back pain (Ermann & Olivier, 2025). People with low back pain have reduced hip mobility compared to those without pain (Eimiller et al., 2026). When one hip is restricted, your lower back compensates. It rotates more, extends more, does jobs it shouldn’t be doing. That’s when pain starts. What I feel when I do this: My right knee drops easily, barely any tension. My left knee? Has a bit of tension and doesn’t drop quite as far. That tells me my left hip is the restricted one. What to do next: Whichever side is tighter needs targeted mobility work.
