Lately I’ve been contemplating whether general practice, as it currently exists, is sustainable for me. In my previous post I spoke about the moral distress of being a GP, constantly torn between wanting to offer a good, thorough consultation and watching the clock tick down ⏰ I once believed GP was the perfect avenue: continuity of care, flexibility, work life balance.. But now, as a seasoned GP with a solid run of experience, I’ve changed my mind. Being a GP is damn hard. We work within a 10 minute appointment system, despite the same BMA contracts recommending 15 minute appointments for safe practice. The reality is many practices simply cannot afford this due to overwhelming patient demand. Within those 10 minutes we are expected to: • take a full history • examine • diagnose • arrange referrals and investigations • prescribe safely, often for patients on multiple medications • safety net red flags And then repeat this process again and again, every 10 minutes, knowing every patient is different. Some take longer to get into the room due to mobility issues. Some come with complexity that simply doesn’t fit into a stopwatch. That level of cognitive load, over and over again, is immense. I haven’t fully decided whether I’ll be leaving general practice altogether. But I do know this: I want to experience acute medicine again. Because being a doctor isn’t the problem. Being a GP isn’t the problem. Conforming to an absurd 10 minute appointment system is. There are many avenues GPs can explore while remaining on the GP register and I’ll be sharing my journey as I work through what’s next 🩺 If you’re a GP who’s struggling: please know you’re not weak, failing, or ungrateful. Many people assume GPs earn far less than hospital specialists. But the reality is more nuanced than people realise. If you’re a medical student considering general practice but worried about income, this is worth understanding. The reality is that GP income is far more flexible than most specialties, and peak earning potential is often reached much earlier. While it won’t match procedural fields like surgery, GP income can be very comparable to many non-procedural specialties. Unlike salaried hospital roles, GP earnings aren’t fixed. They are influenced by factors such as: • Number of sessions worked per week • Billing model (bulk billing vs private) • Patient volume and efficiency • Special interests or procedural work • Practice structure and overheads Some GPs prioritise lifestyle: fewer days, lighter workload, and more balance. Others scale income through extra sessions or niche areas such as skin cancer medicine, women’s health, or occupational medicine. Same specialty. Very different lifestyles and earning outcomes. That flexibility is one of the most attractive aspects of general practice, and why simple “average salary” comparisons can be misleading. GP income is variable, scalable, and lifestyle friendly. Things your GP wishes you knew before your appointment 👩🏼‍⚕️ Appointments are usually short, so these little things can make a big difference: 1️⃣ Bring a list - if you’ve got multiple concerns, ideally book a double appointment so nothing gets missed. 2️⃣ Photos help - rashes or symptoms can change by the time we see you, so photos really help. 3️⃣ Tell us what you’ve tried - creams, painkillers, antibiotics, it all helps us understand what works or hasn’t. 4️⃣ Details matter for kids - exact temperatures, how long symptoms have been going on, and how they’re doing overall. 5️⃣ Be upfront about what you’re hoping for - blood tests, referrals, or specific treatments, we can discuss options together. 6️⃣ Come back if things change - if it hasn’t got better, or you’re concerned, we genuinely want to see you again. 7️⃣ Random questions have a reason - they help us put together a full clinical picture and rule things in or out.
