9 years of USMLE prep. I matched. But looking back, I was training for the wrong finish line. I started this journey in medical school like most IMGs do. Step 1. Step 2. OET. ECFMG. For 9 years, my entire focus was on one thing ,score as high as possible. I believed that a strong score was the answer. And I was not entirely wrong. Scores matter. They get you noticed. They get you interviews. But here is what nobody told me: the moment that interview starts, your score stops speaking. You do. In the 2026 Match, 43.6% of non-US IMGs did not match into a PGY-1 position. Every time that number comes out, the conversation goes straight to visa related issues. But when you look at how program directors actually rank applicants, the top factors are interpersonal skills, how you came across in the interview, and what residents said about you. Not your Step 1. Not your Step 2 or even Visa issues. Your score opens the door. You have to walk through it yourself. Looking back at my 9 years, I over-invested in one thing and under-invested in another. I spent years obsessing over nitty-gritty Step 1 & 2 details mechanisms, minutiae, concepts that would never come up in an interview room. I do not regret working hard. But if I could go back, I would take 20 to 30 percent of that energy and put it into something I almost completely ignored. How to answer a question without rambling. How to tell my story with structure and confidence. How to make a program director feel, in 20 minutes, that they can work with me every day. We have trained ourselves to connect USMLE prep with Step 1 and Step 2 from day one of medical school. That mental link is automatic. It starts early and it runs deep. Communication skills need that same mental link. Not as something you fix at the end.As something you build the same way you build clinical knowledge slowly, consistently, from the beginning of medical school. I genuinely believe that if IMGs treated communication as a parallel track to exam prep from day one, the match rate would move. Not slightly. Significantly. Apply same seriousness as USMLE exam to how you speak, listen and connect. #img #usmle
