Upper GI bleeds can get hectic fast, so here is a quick way to keep things organised on shift. 1. Resuscitate Start with two large bore IVs. Give IV fluids if the patient is unstable and add packed red cells if they are actively bleeding or their hemoglobin is dropping. Always reassess after each bolus or unit. 2. Medications Give an IV PPI early. Add octreotide if you are worried about a variceal bleed. Give ceftriaxone in cirrhosis or if infection risk is high. Keep monitoring vitals and watch the response closely. 3. Call GI for endoscopy Once the patient is stabilised, get GI involved early so they can scope and control the source.
