Stop stretching your neck, if it’s acute cervical radiculopathy, start here instead ⬇️ When someone walks into my clinic with acute cervical radiculopathy, I don’t jump to aggressive stretching or heavy strengthening. These are my go-to early-phase exercises for my patients: 1️⃣Repeated Cervical Retraction (± Extension) ✅ Starting Position * Sit upright in a chair * Feet flat, thoracic spine tall * Eyes level (avoid starting in flexion) 🔹 Movement (Retraction Only) 1. Gently glide your head straight backward (like making a “double chin”). 2. Do not tip your head up or down. 3. Keep eyes level and jaw relaxed. 4. Move to end range without pain peripheralizing. 5. Hold 1–2 seconds, then return to neutral. Reps: 10 repetitions Frequency: Every 1–2 hours in acute phase Stop immediately if symptoms travel further down the arm. 2️⃣ Deep Cervical Flexor Activation (Craniocervical Flexion) Goal: Improve segmental stability and decrease superficial muscle dominance. ✅ Starting Position * Lie on your back * Knees bent * Small towel under head (if needed) * Jaw relaxed, tongue resting gently on roof of mouth 🔹 Movement 1. Perform a small, slow “yes” nod. 2. Think about flattening the curve of your neck slightly. 3. The movement is subtle — not a sit-up. 4. You should NOT feel sternocleidomastoid bulging. 5. Hold 5–10 seconds while breathing normally. Reps: 10 holds Sets: 2–3 🔎 What you should feel * Deep, gentle effort in the front of the neck * No shaking or superficial gripping For cervical radiculopathy: * Mild local neck discomfort is acceptable. * Increasing distal arm symptoms is NOT. * Peripheralization = stop and reassess.
