Even when skin excision is indicated, fat preservation is essential. 🔎 Eyelids are not always the problem. A sad or tired look is often caused by lowered eyebrows, not excess eyelid skin. This can be genetic or age-related, due to weakening of muscles and ligaments. If eyebrows are low, eyebrow elevation should be considered first. 👁️ For the lower eyelids, bone structure and fat volume must be carefully evaluated. In many cases, it is better to reposition fat, restoring volume in the mid-face, rather than remove it. 🚫 Unnecessary incisions on the lower eyelid increase the risk of ectropion, eyelid eversion, and scleral show.
