Cauliflower ear is a condition affecting the outer ear caused by repeated or severe trauma, resulting in the separation of the cartilage from the overlying skin layer. This separation allows blood or fluid to collect between the skin and cartilage, leading to local inflammation and tissue fibrosis. Over time, and without appropriate treatment, this fibrosis hardens, resulting in a permanent deformity of the ear, resembling the surface of a cauliflower. This condition is common among practitioners of combat sports such as wrestling, Brazilian jiu-jitsu, and mixed martial arts, where the auricle is subjected to repeated pressure and friction. The condition arises from traumatic injuries that rupture the tiny blood vessels between the cartilage and skin. This leads to a buildup of blood, which, if not properly drained, transforms into dense, fibrous tissue that alters the shape of the ear. Contributing factors include repeated exposure to direct pressure without protective equipment such as earmuffs. With repeated trauma, the deformity becomes more pronounced and persistent, leading to a loss of the ear's natural appearance and an irregular enlargement. The condition is diagnosed clinically by examining the swollen or deformed ear and a history of repeated trauma. Medical imaging such as magnetic resonance imaging (MRI) or computed tomography (CT) may be used to assess the extent of deep tissue changes if necessary. Differential diagnosis includes relapsing polychondritis, benign tumors such as hematomas, and other inflammatory disorders that may lead to changes in the shape of the ear. Treatment depends on the timing of diagnosis and the severity of the condition. In the early stages, the hematoma can be drained using suction or surgical incision, with local pressure applied to prevent re-accumulation. Compression dressings are recommended to stabilize the skin over the cartilage and promote healing. In chronic cases or when fibrosis has developed, surgical correction may be necessary to reshape the ear and improve cosmetic appearance.
