It took years to diagnose the issue, which relied heavily on compression socks. Treating the perforators, which appeared normal on ultrasound but caused pain, is crucial. The patient’s ultrasound showed issues with the saphenous vein segments, so we treated the connecting veins to save the saphenous vein. We were able to save all three saphenous veins and many perforators, and the patient is expected to recover very well. The patient is prone to lipedema, where high vein pressure inflames the fat, worsening the condition. The key is to treat the veins from the bottom up and inside out. Talk about shallow minds!   Cortical perforators are important for perfusing the superficial cortex along the surface of the brain     Is your knowledge of superficial perforators only skin deep?   If Instagram has taught us anything, it’s that shallow & superficial do matter! 😂   Everyone knows deep perforators, but superficial cortex has perforators too—cortical, subcortical, & medullary twigs to white matter.   ➡️Largest are the lateral perforators along the convexity, while deep medial perforators are small   ➡️Cortical perforators can be affected by emboli or atherosclerosis:
