This was a physical therapy session with one of my patients, an incredible triathlete who just competed at Worlds - so impressive! She’s currently managing a hip labrum tear and hip impingement conservatively. Before Worlds, we were able to successfully calm her hip down, and now we’re focusing on ongoing maintenance as she takes some downtime before her next big season. We’ve had great success incorporating dry needling to her glute max and glute med, which we typically start the session with for 10–15 minutes. This helps “open a window” in the tissues, allowing for more movement before we begin the dynamic mobility and strengthening work. In her sessions, we focus on improving hip rotational control, emphasizing internal rotation (and external rotation as needed) to enhance movement quality and efficiency. At the same time, we strengthen the muscles that support her hip, ensuring she has the stability and power needed to handle the demands of her sport. Key exercises from today’s session: 1. Single-leg hinge → Hip CAR (with groin squeeze on foam roller) – builds rotational control 2. Cable-resisted curtsy lunge off a step – emphasizes hip internal rotation control 3. Side plank hold + banded adduction – strengthens the groin, core, and hip abductors 4. Banded stork to hinge – targets posterior chain and glute med 5. Dealing with SI joint discomfort, tight glutes, or nagging low back pain?
