Rehab for peroneal tendinopathy Peroneal tendinopathy refers to pain and loss of function associated with loading of the tendon. This may be an overuse injury related to activities like running or playing sports, or associated with recurring lateral ankle sprains. It’s not uncommon for chronic ankle instability and peroneal tendon issues to go hand-in-hand. Key targets for rehab are plantar flexion strength, eversion strength and single leg stability during dynamic movements. If you have problems with walking and navigating your daily life, tracking your steps is crucial. It’ll help you determine your baseline level of function and tolerance, so that you can slowly improve both over time. [Outer] Ankle stability + power: PERONEAL edition ✅SAVE THIS & READ IF YOU HAVE ANKLE PAIN! ⬇️ This tendon is usually an issue with most outer ankle pain and after an ankle sprain! The peroneals refer to the 2-3 muscles in the outside of the lower leg that run from the lower leg, down the outer ankle and attach at the base of the pinky metatarsal (brevis) or bottom of the foot near the 1st ray (longus). Peroneus tertius attaches a bit on the front side of the foot to assist with dorsiflexion/eversion (& is not present in ~10-17% of people!) 👉🏼 Structural issues that can contribute to issues here include hindfoot varus, lateral ligamentous instability, an enlarged peroneal tubercle, and a symptomatic os peroneum. The MAIN function of this group is to EVERT the foot and act as an ankle stabilizer. However, brevis/longus ALSO act as secondary PLANTARFLEXORS which a lot of people forget to train. With our calf being a major push off muscle. The peroneus brevis is responsible for ~63% of the power needed to EVERT the foot & assist with plantarflexion with peroneus longus.🤯 Pay attention to this group if you want to run faster & steer clear of ankle issues! 🔗The entire leg must work together from hip to ankle so when we change planes of motion like side steps, or push off an uneven surface, the ankle doesn’t tap out and wobble. ⬇️ these few exercises focus on this group to challenge it accordingly - Ankle eversion PF bias: don’t cheat with the hip/knee. The peroneals refer to the 2-3 muscles in the outside of the lower leg that run from the lower leg, down the outer ankle and attach at the base of the pinky metatarsal (brevis) or bottom of the foot near the 1st ray (longus). Peroneus tertius attaches a bit on the front side of the foot to assist with dorsiflexion/eversion and is actually not present in ~10-17% of people! 👉🏼 Some structural issues that can contribute to issues here include hindfoot varus, lateral ligamentous instability, an enlarged peroneal tubercle, and a symptomatic os peroneum. 👉🏼The MAIN function of this group is to EVERT the foot and act as an ankle stabilizer. (See my ANKLE weakness/CALF strength reel to go with this). 🔗The entire leg must work together from hip to ankle so when we take a side step & abduct the hip, the ankle doesn’t lag in to be subject to rolling inwardly. This is a simple picture, but often what can happen after an ankle sprain when proprioception and strength are impacted. 🔍 It’s important to deduce what exactly is going on in case there is a structural issue, but in any case.. ⬇️ these are a few exercises that focus on this group to challenge it in more ways than simple eversion! 1️⃣ Ankle eversion: in DF AND PF bias: don’t cheat with the hip/knee. In some cases, the patient can voluntarily sublux the tendon, which may or may not be associated with the popping sound. ✅️ Surgical management is generally warranted. The postoperative AOFAS score, VAS scale, and high satisfaction percentages all improve with surgical therapy.
