If perfusion is critically impaired, no dressing will fix the problem. Lower limb wounds that are not healing should always trigger one question first: Is the blood supply adequate? 🚨 Rest pain 🚨 Pallor on elevation 🚨 Dependent rubor 🚨 Non-healing ulcer with dry black eschar 🚨 ABPI < 0.5 🚨 Toe pressure < 30 mmHg 🚨 Sudden neurovascular change These are not “review next week” findings. They are urgent vascular red flags. Compression without adequate arterial supply can cause harm. Assess perfusion. Escalate early. Time is tissue. Save this for your next lower limb assessment. A dislocated knee is extremely painful and should always be treated as a medical emergency. In some cases, the joint may slip back into place on its own or with gentle movement, but that doesn’t mean the injury is minor. Even when the knee “pops” back in, there can be serious damage to ligaments, cartilage, nerves, or blood vessels. Sometimes careful extension or flexion may reduce the dislocation, but attempting to realign it without medical supervision is risky. Swelling, instability, numbness, or severe pain are warning signs that need immediate evaluation. Imaging tests are often required to rule out hidden injuries. Prompt medical care is crucial to prevent long-term complications and ensure proper healing. #kneeinjury #medicalhelp #orthopedics #injuryprevention

To all my colleagues in orthopedics, if Cardiology is a walking red flag, then you are a walking check engine light. Hope this helps.

When seconds matter and IV access isn’t happening, IO access gives you rapid, reliable vascular access through the bone marrow. 🦴 When to use it: Cardiac arrest, shock, trauma, sepsis—any time IV access is delayed or unsuccessful. 💉 What can go through an IO? Fluids, blood products, and all emergency meds. IO = IV equivalent in resuscitation. 📍 Common sites: Proximal tibia, distal tibia, proximal humerus (great flow rates in adults). ⚠️ Key nursing considerations: • Landmark carefully • Flush aggressively after placement • Use pressure for fluids • Treat pain with lidocaine in awake patients ⏳ Remember: IOs are temporary access. Transition to IV or central access as soon as possible. Knowing when to place an IO—and being confident doing it—can save a life. Stitch w/ @anselmo13simba on TT

@emswami and I highlight how emergency clinicians excel at resuscitation, rapid task switching, and risk stratification, especially in rural or community hospitals without on-site specialists. While ED teams may not match a specialist’s precision in procedures like deliveries or reductions, they often perform them exceptionally well and far beyond most physicians, because it’s part of the job. A key skill is knowing when to call for help or escalate care, while recognizing that sometimes even consultants can’t solve the problem in the moment, underscoring the limits of any clinician in certain situations • #emergencymedicine #medicine #medicaleducation

Once we find a leak in the Wound VAC seal, we patch it using a trimmed piece of drape. With KCI drapes, it’s easy—they’re numbered for step-by-step application. Secure side one, peel off side two, remove the blue tab, and just like that—the seal is fixed. No more air leaks, no noise, and most importantly, no surprises at home. This is a hospital bed. And while it’s essential for healing, spending too much time lying flat can quietly work against recovery by increasing the risk of blood clots, skin breakdown, cardiovascular issues, and even bowel paralysis. Our bodies are built to stand. To move. To stay upright. That’s why our nursing teams work so hard to get patients up and walking, and why we’ve started using wearable tech like Fitbits to help. With step counts displayed right in the hospital room on the TV set, movement becomes visible… even a little gamified. And when patients who can walk end up walking more, they may recover faster and even get home sooner. Sometimes innovation isn’t about doing more tests. Here, it’s just about about helping people get up and move.
