I’ve spent years watching patients make decisions that cost them decades of healthy living because nobody told them what actually matters. Traditional healthcare checks boxes and calls it prevention. Insurance companies decide what tests are necessary based on cost, not outcomes. Most people trust their annual physical is keeping them healthy when it’s really just confirming they’re not sick yet. Here are the four hills I’ll die on about living to 90, even if they make other doctors uncomfortable. Hill #1: Your annual physical is not preventative care. Most annual physicals check if you’re sick right now. They don’t tell you what’s building under the surface or what you can do to stop it. Real preventative care measures visceral fat, VO2 max, fasting insulin, and coronary calcium scores. It catches risks years before symptoms appear. Hill #2: VO2 max matters more than the number on the scale. Your weight tells you almost nothing about your actual health. VO2 max is your body’s ability to use oxygen during exercise, and it correlates directly with lifespan and healthspan. Someone thin can be metabolically unhealthy while someone carrying extra weight can have excellent cardiovascular capacity. Hill #3: Most people aren’t healthy. They’re pre-sick. Normal lab ranges are based on averages, not optimization. Just because your fasting glucose is under 100 doesn’t mean your insulin sensitivity is where it should be. Most people are walking around with silent metabolic dysfunction that won’t show up on standard testing until it’s progressed. Being told you’re fine and actually being optimized are two different things. Hill #4: If you wait until your 60s to care about longevity, you’ve already lost a decade. The decline that shows up in your 60s starts in your 40s through gradual losses in muscle mass, cardiovascular capacity, and insulin sensitivity.
