Patient Details: Age: 35 years Gender: Male Clinical Presentation: A slowly growing, painless swelling over the left lateral thigh for approximately 8 years. Ultrasound Findings: A well-defined, oval-shaped hypoechoic cystic lesion is seen within the dermis of the left lateral thigh. The lesion appears superficially located and mildly displaced inferiorly. A central hyperechoic thickened focus is noted, consistent with a keratinous plug. Posterior acoustic shadowing is present beneath the central echogenic component. On Color Doppler examination, there is no internal or peripheral vascularity. No evidence of extension into subcutaneous tissue, muscle, or deeper planes. Impression: Ultrasound features are suggestive of a Epidermoid cyst (also known as sebaceous cyst clinically). No sonographic evidence of malignancy or deep tissue invasion. 📖 Definition Epidermoid Cyst (Sebaceous Cyst) An epidermoid cyst is a benign, slow-growing cyst that arises from the epidermis. It contains keratin material and often shows: Well-defined margins Hypoechoic appearance Central echogenic keratin plug No vascularity on Doppler Posterior acoustic shadowing (due to dense keratin) These cysts are usually painless unless infected.

Did someone cast a furnunculus spell on this poor lady🧙‍♀️? This is one ugly spot 😆. Most seborrhoeic keratoses hang out with some friends, but, just like human beings, some of them are not at all sociable. This one was hanging out on its own, and seemed to be thriving on the solitude! In general, rapid growth of a skin lesion is worrying for malignancy, but this one looks like a sebK and was confirmed to be so by the pathologist. Large epidermal cyst removed seamlessly — no scar, just smooth healing. ✨

It’s been a while since I’ve shown you an impossibly small skin cancer. This one is tops 1.5ish mm in size! But that pearl appearance caught my eye. There are a few things that can look like this with the naked eye including: Basal cell cancer (of course!) Sebaceous hyperplasia (big sweat glands) And Trichoepithelial cysts (a harmless cyst that looks just like some basal cell cancers at times!) Dermoscopy helps greatly in being able to tell which ones need our attention and which ones don’t. Because it’s been found nice and early this gent will get away with a much smaller surgery than if we’re left out to grow and in that location every mm counts!!!!
