Rhinoplasty: one year post-op. Nasal tip stabilization is the most critical aspect for having a good longterm result. One key maneuver for tip stabilization is nasal tip deprojection to create less leverage and less torque and therefore greater stability. Longterm results matter the most in rhinoplasty. Revision Rhinoplasty: This patient had a previous rhinoplasty which left her with a poorly supported drooping tip, an over-projected tip, collapsed lateral crus, and nostril asymmetry. This case demonstrates the ability of nasal tip deprojection to provide greater support of the tip and the complex structures, including the lateral crus. Nose casting after rhinoplasty is a crucial step in shaping and protecting the new nose. In this ethnic rhinoplasty case, rib cartilage was used to build a strong and long-lasting nasal framework. Revision Rhinoplasty: one year post-op. This patient had a previous rhinoplasty which left her with a poorly supported drooping tip, an over-projected tip, and a bulbous tip. She is now one year post-op from revision rhinoplasty with MTF cartilage and a mastoid fascia tip graft. She underwent repositioning of her lower lateral cartilage to stabilize her tip, spreader grafts, extended alar contour grafts to help improve nostril symmetry, tensioning of the tip to help correct alar retraction, and nasal tip deprojection.  The goal is a natural looking result which complements the rest of her face and does not droop when she smiles. . .
