I had just come out of a big epileptic seizure before even starting my day. I was too groggy to notice an alert from Echo — and Ruben was too busy cleaning up the mayhem from the first seizure to notice. Typically, Echo barks to alert. But when human help is present, he’s taught to alert quietly and stay out of the way. He lays on top of me to keep my joints from dislocating as this is so common with Vascular Ehlers Danlos Syndrome. Ruben didn’t get to film the first seizure and the home cameras were charging. From what I heard, it was sudden and chaotic. Arteriovenous fistula (AVF) is an abnormal connection between an artery and a vein, allowing blood to flow directly between them without passing through the capillaries. This connection can be congenital or acquired, either due to trauma or surgical procedures. In patients with chronic renal failure, this fistula is surgically created to provide a suitable vascular access for hemodialysis, which contributes to improving the efficiency of the procedure and reducing complications associated with central venous catheterization. Clinical symptoms result from the abnormal shunting of blood, as direct arterial flow leads to excessive venous dilatation and increased local venous pressure, which may cause edema and increased pulse rate in the affected limb. Physical examination often reveals a palpable quiver over the fistula, in addition to a continuous murmur that can be heard with a stethoscope, reflecting turbulent blood flow. Doppler imaging may show increased flow within the dilated vein, confirming the diagnosis. Patients on dialysis are at highest risk for developing this condition, given the surgical construction of their connections. However, acquired arteriovenous connections can occur after traumatic injuries or medical procedures involving blood vessels. Predisposing factors also include chronic vascular disease, such as atherosclerosis, which may increase the likelihood of complications after vascular surgical interventions. Management is based on clinical and radiological assessment, and may require surgical intervention in cases that cause severe symptoms or complications such as heart failure due to hemodynamic overload or ischemia in the affected limb. In uncomplicated cases, periodic monitoring may suffice to ensure stable blood flow and no complications.
