Macular edema. What is it? 👁️ Macular edema is fluid buildup in the macula - the central, high-resolution part of your retina responsible for sharp vision, color vision, and fine detail. When fluid leaks into this area, it causes swelling and distortion. Many patients describe it as looking through water or warped glass. The core issue is usually a breakdown of the blood-retinal barrier, along with inflammation and poor circulation. Here’s what’s happening underneath: 🔹 Leaky blood vessels Retinal capillaries become permeable. Fluid, proteins, and sometimes blood seep into the macula, often driven by VEGF and oxidative stress. 🔹 Inflammation Inflammatory chemicals increase vascular permeability and weaken the protective barrier. 🔹 Impaired fluid drainage Müller cells and the retinal pigment epithelium fail to pump fluid out efficiently. 🔍A 70 year old gentleman presented with sudden loss of vision over the weekend in his left eye. He has a strong cardiac history with a previous quadruple bypass. What do the following images show?🧠 The images reveal the presence of a pre-retinal haemorrhage, sub-ILM haemorrhage, IR haemorrhage (not seen on this OCT) and sub-retinal haemorrhage. The most likely diagnosis was a ruptured retinal macroaneurysm and he was referred urgently to the VR team who listed him for vitrectomy. Retinal macroaneurysms occur when the walls of retinal arteries become weak and can rupture (as in this case) leading to sudden and dramatic vision loss. Typical risk factors include raised blood pressure, atherosclerosis, ageing and diabetes. The prognosis in this case is guarded due to the extent of the bleed and as it has affected the macula. #optometry #eyecare #news #eyeeducation #study
