Conventional imaging like X-ray, MRI, and CT scan showed no clear abnormality—because they only capture static images. However, using Digital Motion X-ray (DMX), real-time imaging under motion revealed severe upper cervical instability at C1, where the head was no longer being properly supported by normal ligamentous structures. Instead, it showed extreme instability and loss of structural support, consistent with significant ligamentous laxity. Critical Atlas Adjustment Helps Neck Pain & Stiffness! What is the critical atlas adjustment? For those of you new to my socials, I will explain the critical atlas adjustment here. When I say the patient has a critical atlas, it means that there is an atlas (C1) misalignment on one side and there is an opposite misalignment of C2. Basically, C2 has an opposite wedge to C1. In the gonstead method, the way we adjust the atlas is on the side of laterally, the side of the open wedge. It is a lateral to medial adjustment setting the atlas across both lateral masses over C2. Now if C2 is wedge on the opposite side, there won’t be a stay put value of the adjustment. So, C2 (the bottom segment) is adjusted first to give C1 a better foundation to be adjusted on. The rotational component of the C1 misalignment is taken out during positioning of the patients neck prior to the adjustment. For example, if the atlas has an anterior rotational component then we turn the head towards the doctor to remove the anterior rotation. Conversely, if there is a posterior rotational component to the misalignment then the head is turned away from the doctor to remove the posterior rotation.
