For a long time, cardiogenic shock has carried a grim 50% survival rate — a coin toss between life and death. When the heart can’t pump effectively, other organs begin to shut down, and things can deteriorate quickly. I went into cardiogenic shock due to acute viral myocarditis — which is just a medical way of saying a virus attacked my heart and caused severe inflammation. During those critical hours in the ER and ICU, the chances of me coding were very real. My cardiologists determined I needed immediate support to survive. They implanted the Impella heart pump, a device that temporarily takes over the heart’s workload so it can rest. At one point, we were told a heart transplant might be necessary — but doctors wanted to give my heart just a little more time to rest and see if it could recover. Thankfully, my other organs held steady. After two weeks with two different Impella devices, my ejection fraction (which measures how much blood the heart pumps out with each beat) improved from less than 10% to 45% — incredibly close to normal range (50–70%). My heart had recovered. The Impella pump was removed, and just a few days later, I was discharged home. Within one year, I was weaned off all my medications — blood thinners, heart meds, blood pressure meds — all of it. Today, I no longer have heart failure. I have no lasting damage. My heart made a full recovery, and I could not be more grateful.

Sharing a case I just reviewed - Complications happen to every operator if you do enough procedures, but what matters most is how you respond to an emergency. Coronary perforations are undoubtedly one of the most feared complications in the cath lab. Risk is higher for heavily calcified lesions, tortuous lesions, using hydrophilic wires, excessive post dilation, and in older female patients as well. If it happens: 1. Stay calm. 2. Stay calm. 3. Call for an echo tech, pericardiocentesis tray, and covered stent to be on deck. 4. Quickly inflate a balloon 1:1 size of the vessel for serial inflations at the perforation site, usually 3-5 minute inflations x3. 5. In this time ask for a bedside echo to determine whether you need to do a pericardiocentesis to remove the fluid. Assess the rv, IVC etc for signs of tamponade. 6. If angiogram shows persistent perforation after serial balloon inflations deploy a covered stent (papyrus or graftmaster).
