And we listened. Our son toe walked when he was little. It didn’t seem urgent. It didn’t seem scary. Until it wasn’t just toe walking anymore. The speech delays came. The skin flares showed up. If a child was speech delayed at age 2, here are the 4 things I’d do immediately as a BCBA-D, autism mom and RN: 1️⃣ Get super clear on the child’s strengths & needs. I’d look at what sounds, gestures, eye contact, and play skills they do have so we can build from there. Assessment is always the first step! 2️⃣ Boost connection before communication. Be the fun person—give the child the things they love while repeating the single target word 3 times. Follow their lead, keep activities fun and short, and celebrate any attempt to engage. 3️⃣ Teach 1–3 functional words based on their favorite things. Skip long lists. Choose highly motivating words like “ball,” “bubbles,” or “milk”—whatever your child loves most—and model them naturally throughout the day. 4️⃣ Reduce frustration proactively. Signs of autism can start to become obvious for some kids as early as 12 months old, but in my experience they start to become clearer and more obvious by about 18 months. These are the signs of autism we saw in E at 18 months and what ultimately led to him being diagnosed 3 months later! Autism is a social communication disorder and a developmental disability. It isn’t a collection of “quirks.” If you’re worried about your child’s development, please speak with their pediatrician! ▪️Lack of name response - It genuinely seemed like E had no idea what his name was. It wasn’t that he was ignoring us, he actually did not realize we were saying his name. ▪️No pointing, few gestures - This was the biggest sign for us! E’s only gesture he used to communicate was reaching to be picked up. So not only was he nonverbal (and still is) but he couldn’t communicate nonverbally either. Pointing is not delayed until 18 months though! ▪️Constant repetitive movements - when I say constant, I mean CONSTANT. E wouldn’t just spin a few times a day, he would sometimes spin for 20-30 minutes at a time, until he was literally falling over. Then he’d get back up and do it again! The vast majority of his play was repetitive movements and we could not distract him from this. An attempt to redirect would turn into a meltdown. ▪️Developmental delays - autism is a developmental disability. Developmental delays in more than just social communication are common in autistic kids. E was (and still is) delayed in nearly every area of development.
