Refazer o exame físico do joelho, após a anestesia e antes do início dos procedimentos cirúrgicos, me ajuda a entender melhor a instabilidade da articulação. O teste do Pivot-shift, geralmente é difícil de ser realizado no consultório mas fica evidente sem a ação muscular do paciente, como mostrado no vídeo. O teste é realizado com o paciente em decúbito dorsal e extensão completa do joelho. O examinador faz rotação interna da tíbia, estresse em valgo do joelho e parte para a flexão. Quando o teste é positivo (como no vídeo) a tíbia, subluxada em redução ao fêmur, reduz. Have you ever heard of the Lever Sign Test aka the Lelli’s test to check the integrity of the ACL? Clinicians tap in and let me know what you think of this test / if you use it below 👇 Negative test: When the ACL is intact the downward force applied over the quadriceps will cause the heel to rise. Positive test: With a damaged ACL the downward force will cause anterior translation of tibia in relation to the femoral condyle. So, the heel will not rise! I think I read somewhere that it has incredibly high sensitivity and specificity and from clinical experience it’s been right for me 100% of the time. Why use this? it’s one of the EASIEST tests you can do as a clinician & it’s way easier to get the patient to relax to get an accurate result especially when compared to a Lachman’s or Pivot Shift test. We still also used the Anterior Drawer test for this patient which was also positive just FYI I will even have patients do this on themselves when I suspect an ACL tear over telemedicine calls to rule in/out my suspicions. The pivot shift test isn’t scary, it isn’t too painful, and yes, patients can relax. In my 17 years of working with acute knee injuries, I know many clinicians avoid it. Why? 👉 Lack of confidence in how to perform it 👉 Uncertainty in how to interpret it 👉 Being told it’s too provocative and should only be done by an ortho surgeon (not true!) Some still believe you can only do it under anesthesia. Also not true. IF you understand the mechanics of the test, position your patient correctly, and execute it with confidence, it’s not a painful or aggressive test and patients can tolerate it very well. I’ve taught dozens of clinicians how to confidently perform the pivot shift test, and I’ve seen plenty of “AHA!” moments when they realize it’s not as intimidating as they thought. Knee Ligament Testing . Nothing crazy complex here, but hopefully will be helpful! Sorry my captions weren’t working today! . MCL and LCL I like to check in full extension and slight knee bend. If loose in full knee extension the tear could be more significant. . For ACL, we have anterior drawer and Lachman’s. We could do Pivot shift, but I find these 2 enough often. . For PCL, not shown, but you could do a posterior drawer test. Normally you just need to assess their overall tibial position though and it will be resting posteriorly. . Why no meniscus tests? Normally with a meniscus tear, if you’ve ruled these ligaments in or out, a meniscus tear is normally going to have a block in ROM either in flexion or extension, have noticeable swelling typically, may have catching symptoms etc. The point is, I don’t think you really need meniscus tests, it’s more so a clinical appearance with usually standard mechanisms (twisting etc). I guess I also labelled this “Ligament Testing” too. 😂 . Thanks for watching! Please share if helpful!

Até recentemente, o teste de LACHMAN sempre foi o que apresentou maior acurácia diagnóstica nas rupturas parciais ou totais do LCA, com sensibilidade variando de 80% a 90% na literatura. 🔸Porém, na última revisão sistemática (2022) sobre o tema, publicado na Knee Surgery, Sports Traumatology & Arthroscopy, verificou-se que o teste/sinal de LEVER apresentou sensibilidade SUPERIOR ao Lachman, tanto nas lesões agudas (<3 semanas) como crônicas (>3 semanas). Segue abaixo os achados: 📌 PIVOT SHIFT: Sn: 48% Sp: 96% 📌 GAVETA ANTERIOR: Sn: 75% Sp: 92% 📌 LACHMAN: Sn: 85% Sp: 93% 📌 LEVER: Sn: 98% Sp: 93% (Sn: sensibilidade / Sp: especificidade) Conclusões: ✅ Até o momento, individualmente o sinal de LEVER parece ser o melhor para diagnóstico clínico das rupturas do LCA ✅ O Pivot Shift tem alta ESPECIFICIDADE, ou seja, é excelente para descartar possíveis rupturas do LCA ✅ Cada vez mais a tendência é utilizarmos CLUSTER de testes para aumentar a acurácia do diagnóstico.
