🎯 Allen Testi 📍 Allen testi, eli besleyen arterlerdeki (radial ve ulnar arter) kan akışının açıklığını ve yeterliliğini değerlendirmek amacıyla kullanılan, basit ve invazif olmayan bir klinik testtir. ✍️ Endikasyonları 📍 Allen testi, özellikle önkoldan yapılan arteriyel girişimler veya arter grefti alınması planlanan durumlarda uygulanır. 🔹 Periferik damar hastalıklarının değerlendirilmesi 🔹 Koroner arter bypass cerrahisinde greft amacıyla radial arterin kullanılabilirliğini belirlemek 🔹 Arteriyel kateterizasyon (ör. kan gazı ölçümü veya invazif basınç monitörizasyonu için radial arter kateterizasyonu) öncesinde güvenliği değerlendirmek ✍️ Uygulanışı 📍 Hasta elini yukarı kaldırır ve birkaç kez sıkıp açarak venöz dolum artırılır. Ardından el sıkılı tutulur. 📍 Muayene eden kişi aynı anda radial ve ulnar arterlere basınç uygularak kan akışını geçici olarak durdurur. Bu sırada elde solukluk (beyazlaşma) gelişir. 📍 Sonrasında radial arter üzerine basınç sürdürülürken, ulnar arter üzerindeki basınç serbest bırakılır. Böylece kanın ulnar arter yoluyla ele yeniden dolması beklenir. 📍 Normal şartlarda 5–7 saniye içinde eldeki cilt rengi normale döner. ✅Eğer renk bu sürede geri dönerse test negatif kabul edilir → Ulnar arter dolaşımı yeterlidir. ❌Eğer renk geri dönmez ya da belirgin gecikme olursa test pozitif kabul edilir → Ulnar arter dolaşımı yetersizdir. 🔎 Dipnot; 📌Negatif Allen testi, radial arter girişimlerinin (kan gazı, kateter, greft) güvenle yapılabileceğini gösterir. 📌Pozitif Allen testi durumunda, sadece radial arterin kullanımı ciddi iskemi riski doğurabileceğinden girişim yapılmamalıdır. ⚠️Testin subjektif olması nedeniyle, şüpheli vakalarda Doppler ultrasonografi gibi objektif yöntemlerle damar açıklığı değerlendirilebilir. . . . . #arter #nabız #112 #anestezi #kalpdamarhastalıkları #kardiyoloji #emergency #112 #acilservis #paramedic #hemşire #nurse #nurselife💊💉 #112acil #doktor #doctor #radial #ulna #ulnar

Kübital Tünel Sendromu(Ulnar Sinir Sıkışması) 💫Kübital tünel sendromu (CBTS) periferik sinir sıkıştırma sendromudur. Dirsekteki kübik tüneldeki ulnar sinirin tahriş veya yaralanmasıdır. 💫Buna ulnar sinir sıkışması da denir ve karpal tünel sendromundan sonra üst ekstremitede en sık görülen ikinci kompresyon nöropatisidir. 💫Hastalar sıklıkla hem duyusal hem de motor defisitlerle başvurur, ikincisi daha az olumlu bir prognozla uyumlu geç bir prezentasyon gösterir. ⭕️Kübital tünel sendromunun en yaygın belirtileri şunlardır: 💢Yüzük ve küçük parmağında uyuşma ve karıncalanma, ulnar sinir sıkışmasının ortak belirtileridir. Genellikle bu belirtiler anlık gelir ve gider. Dirsek büküldüğünde, örneğin telefonu tutarken daha sık görülürler. 💢Tutuşun zayıflaması ve parmak koordinasyonunda zorluk (bir enstrüman çalmak gibi) ortaya çıkabilir. Splint adjustments are very common with ulnar gutters because of complaints of discomfort over that pesky ulnar styloid. Use old putty to give the ulnar styloid a little breathing room by molding a space over the area. I use two short strips of stockinette to put the putty in between - this way the putty doesn’t touch or damage the full length stockinette or the orthosis itself. The putty in the stockinette can be easily discarded following molding. The Brachial Plexus Tension/Elvey Test, also known as Upper Limb Tension Tests, are designed to put stress on neurological structures of upper limb by elongating them. ULTT are performed to assess peripheral nerve mobility and compression or as a part of neurodynamic assessment. These tests are performed as a cluster to make a confirmatory diagnosis for nerve involvement. They are both diagnostic and therapeutic. If you have wrist pain it’s important you don’t immobilize your wrists. Instead try these movements to strengthen them depending on where the pain is. Most importantly don’t work thru pain only go as far as it doesn’t hurt. Then back off. Pain causes the opposite of healing. Even if you can only do micro movements. As long as it’s not in pain.
