✴️Claw Hand Deformity✴️ 👉This is how a claw hand occurs with ulnar nerve dysfunction: 💥Intrinsic function of the ring and small fingers (ulnar n. Innervated) is minimized so 🚫 MP flexion and 🚫 IP extension is present 💥At the MP joint the EDC is now unopposed and hyperextension occurs 💥At the IP joints the FDP pulls the IPs into flexion (assuming the FDP is not fully affected by the ulnar n. Lesion) 💥The wrist will sometimes automatically flex as well! ✌️The index and middle fingers are not involved because the lumbricals of these digits are usually innervated by the median nerve and thus maintain some intrinsic function. ▪️Claw hand is most notable with a low ulnar nerve lesion because the FDP of the small and ring fingers is still intact. In a high ulnar nerve lesion, claw hand may not be as noticeable. ⭕️ One of the ways to address this is with an anti claw orthosis. It prevents the MP hypertext/IP flex posture and with the MP slightly flexed, redirects the extension to the PIP joints. ▪️If the damage is severe there may not be any intrinsic function at all, in which case the orthosis might not help. But here there is some intrinsic function, it’s just weak.
