‼️ Post-exertional malaise (PEM)‼️ PEM is something which the ME community is all too familiar with. It’s a hallmark of ME, but is unfortunately also an experience which many Long Covid patients face. But what is it? Any exertion, however minor it might seem to a healthy person, can lead to a worsening of symptoms and a reduction in functional ability in someone with ME/Long Covid. That exertion could be physical, cognitive, emotional or social. For some, that exertion might be a 1 hour cup of tea with a friend, for others it might be a hospital appointment, it could be an emotional outpouring at the difficulty of the condition, or it could be something like going to the loo or picking up your phone. It’s hard to know when PEM will occur, making it very difficult to manage. What’s worse is that the worsening of symptoms comes on in just hours or days, but improving back to “baseline” generally takes much longer. It’s very difficult to explain PEM to a healthy person, and it’s hard to try to capture the essence of it in just 1 minute. But this is my attempt to explain just how debilitating it is. In fact it is so unbelievably bad that for years it just wasn’t believed.
