A young patient walks in with chest pain. The ECG lights up with ST elevation. Everyone freezes. Is this a STEMI… or is this Benign Early Repolarisation? BER is one of the most common ECG patterns you’ll ever see, especially in young, healthy patients. There are two defining ECG features. First: You’ll see diffuse, concave ST elevation, most prominent in the precordial leads, usually less than 2 millimetres. Second - and this is the key examiner favourite: J-point elevation with notching or slurring. That notch is often clearest in V4, and may look softer or slurred elsewhere. Now the safety rule that matters worldwide: There are no reciprocal ST depressions. If you see reciprocal changes, this is not benign. Context matters. Be cautious calling BER in patients over 50. In that group, ischemia moves much higher up the list. And troponins? Real life is messy. Some clinicians take one. Others insist on serials. The principle is simple: the ECG pattern must agree with the clinical picture. So the takeaway - in any country, any exam, any ED: Concave ST elevation. J-point notching. No reciprocal changes.
